Certidão

 Below are the documents for the request for Certidão:

  1. Request form (please fill-up information), one (1) photocopy

  2. Two (2) photocopies of valid passport Data page and RNM ID of applicant

  3. Two (2) photocopies of PSA birth certificate that shows the full names of both parents

Kindly e-mail the documents to brasiliape.consular@dfa.gov.ph for processing and evaluation. Once approved, you may submit your documents in person or through mail. The Embassy assumes no liability for any loss of documents once they have been handed over to the courier service.

If applied with the Honorary Consulate, please coordinate with them directly regarding the process and payment.

FEES (Only pay once the documents are received by the Embassy for processing)         

Once paid, please email the proof of payment to brasiliape.consular@dfa.gov.ph
