The requirements for the notarial of the affidavit of guarantee and support are:

1. Accomplished Affidavit of Guarantee and Support, a sample is attached for reference (1 original, 1 photocopy);
2. Letter of explanation as to the purpose of the invitation and relationship with the invitee (1 original, 1 photocopy);
3. Copy of inviter's passport data page and contact details (2 copies);
4. Copy of invitee's passport data page and contact details (2 copies);
5. Inviter's Employment Certificate/Company Letter (in English) signed by Human Resource or Employer. Please provide a copy of the valid ID or passport of the signee. (2 copies); and,
6. Inviter's Proof of Income for the last 3 months (Bank statement/copy of Bank Book/Pay Slip) (2 copies).

The consular fee is US$25.00 or R$131.25.
